Connected Heart Care Pathway is a new and innovative service spanning Camden and Haringey

It aims to help patients manage heart failure better by creating a more optimal pathway of care between GPs, hospitals, community services and other organisations involved in heart care.

Vision and goals

To ensure more people with heart failure are correctly diagnosed
Train and educate clinical staff on heart failure

Help patients manage their condition remotely, all the while increasing their self-confidence in managing their own condition

Explore the use of an EKO stethoscope, which uses AI to help clinicians diagnose patients with heart failure

When did the service launch and where is it based?

The service launched at the end of July 2024.

For now it operates in North West and South West Haringey, as well as North Camden.

How are patients remotely monitored?

We use a clinical software called Ortus iHealth.

What is the referral process?

We are a proactive service so not presently accepting referrals.
