About the service
The Haringey GP Federation is rolling out a new digital platform called ‘MyWay Digital Health’ across Haringey. This online platform will help patients better manage their diabetes through personalised dashboards and structured education courses.
All diabetic patients living in Haringey are eligible to access this platform.
How does the platform work?
The platform is free to use and offers a range of benefits for patients, including:
- Access to personalised dashboards with key information such as latest test results and upcoming appointments.
- Opportunities to take part in educational courses that support effective diabetes self-management.
- Availability in many languages, helping improve accessibility and equity across multiple communities.
Who is the service for?
All patients living in Haringey with diabetes can access this platform.
How do referrals work?
When newly diagnosed, a clinician will refer patients to the platform. The patient can then follow the instructions provided to sign up.
Patients who have already been diagnosed with diabetes can self-register. These patients should speak to their GP if they would like more information.
