NHS Health Check Invitation

Dear Patient,

You are invited to attend a free NHS Health Check at your GP practice. This is part of a national programme to help prevent heart disease, stroke, type 2 diabetes, kidney disease, and certain types of dementia.

As you are aged between 40 and 74 and do not have a pre-existing condition, you are eligible for this health check every five years.

What to Expect

The Health Check will take about 20–30 minutes, and during the appointment, we will:

• Ask about your lifestyle and family history
• Measure your height, weight, blood pressure, and cholesterol
• Offer advice and support to help reduce your risk of developing serious health problems

To book your NHS Health Check, please

• use the self-booking link provided in the text message or
• contact Haringey GP Federation on 0203 074 2710,  if you are unable to book through the self-booking link.

• Feel free to let’s know if you require translations or interpretations
• We strongly encourage you to attend this check, as it can help you stay healthy and spot any early signs of potential problems.
