Neighbourhood Health

Helping people get the right support closer to home

Neighbourhood health is about making it easier for people to get the care, advice, and support they need in their local community.

Health services, social care and community organisations are working more closely together so that people get joined-up support that focuses on what matters to them.

In Haringey, we are testing a new way of working by creating three neighbourhood teams to support local residents – the Central, East, and West Integrated Neighbourhood Teams.

We are building on the existing strong working relationships developed by Haringey's multi-agency care coordination (MACC) team.

Initially, the neighbourhood teams will focus on adults with complex needs – those living with frailty or multiple long-term health, care and social needs – who are identified as needing extra support. The teams will then expand their support to the wider community.

By supporting Haringey residents to make healthy choices and stay well, we aim to reduce avoidable long-term health conditions and help residents across the whole borough enjoy more years of good health.

What are neighbourhood teams?

Integrated neighbourhood teams bring together professionals from different services in one local team.

This could include GPs, nurses, social workers, mental health specialists, pharmacists, physiotherapists and community organisations.

Instead of having to contact lots of different services, people will be supported by a team that works together and helps them find the right care and support.

The teams can help with a wide range of issues, including:

  • physical health
  • mental health and wellbeing
  • housing
  • employment
  • benefits and financial support
  • connecting with local community groups and activities

What does this mean for Haringey residents?

We want to spot problems earlier and provide support before issues become more serious.

This means:

  • helping people stay healthy and independent for longer
  • making it easier to get support close to home
  • improving people’s experience of health and care services
  • reducing avoidable trips to A&E and hospital stays
  • helping people live healthier lives and enjoy more years in good health

By working together, we can support people before they reach crisis point and make sure they get the help they need when they need it.

Who is leading this work?

Neighbourhood health is being developed by Whittington Health NHS Trust, Haringey Council and Haringey GP Federation, working with other local health, care and community organisations.

We will continue to listen to residents, patients and carers so that services are designed around the needs of local people.

Our neighbourhood approach is in line with the West and North London 10-year strategy and the Integrated Care Board has invested in our work: 10-year strategy.

Why are we making these changes?

Health and social care services are facing increasing pressure, especially in hospitals and A&E departments.

We need to do more to help people stay well, prevent illness where possible, and provide support earlier and closer to home.

The aim is to help people get the right support, from the right service, at the right time, whether that is healthcare, social care, housing advice, mental health support or help from a local community organisation.
